What is ROOT CAUSE ANALYSIS? What does ROOT CAUSE ANALYSIS mean? ROOT CAUSE ANALYSIS meaning

What is ROOT CAUSE ANALYSIS? What does ROOT CAUSE ANALYSIS mean? ROOT CAUSE ANALYSIS meaning - ROOT CAUSE ANALYSIS definition - ROOT CAUSE ANALYSIS explanation. Source: Wikipedia.org article, adapted under https://creativecommons.org/licenses/by-sa/3.0/ license. Root cause analysis (RCA) is a method of problem solving used for identifying the root causes of faults or problems. A factor is considered a root cause if removal thereof from the problem-fault-sequence prevents the final undesirable event from recurring; whereas a causal factor is one that affects an event's outcome, but is not a root cause. Though removing a causal factor can benefit an outcome, it does not prevent its recurrence with certainty. For example, imagine a fictional segment of students who received poor testing scores. After initial investigation, it was verified that students taking tests in the final period of the school day got lower scores. Further investigation revealed that late in the day, the students lacked ability to focus. Even further investigation revealed that the reason for the lack of focus was hunger. So, the root cause of the poor testing scores was hunger, remedied by moving the testing time to soon after lunch. As another example, imagine an investigation into a machine that stopped because it overloaded and the little fuse blew. Investigation shows that the machine overloaded because it had a bearing that wasn't being sufficiently lubricated. The investigation proceeds further and finds that the automatic lubrication mechanism had a pump which was not pumping sufficiently, hence the lack of lubrication. Investigation of the pump shows that it has a worn shaft. Investigation of why the shaft was worn discovers that there isn't an adequate mechanism to prevent metal scrap getting into the pump. This enabled scrap to get into the pump, and damage it. The root cause of the problem is therefore that metal scrap can contaminate the lubrication system. Fixing this problem ought to prevent the whole sequence of events recurring. Compare this with an investigation that does not find the root cause: replacing the fuse, the bearing, or the lubrication pump will probably allow the machine to go back into operation for a while. But there is a risk that the problem will simply recur, until the root cause is dealt with. Following the introduction of Kepner–Tregoe analysis—which had its origins in the highly complex arena of rocket design, development and launch—RCA arose in the 1950s as a formal study by the National Aeronautics and Space Administration (NASA) in the United States. New methods of problem analysis developed by NASA included a high level assessment practice called MORT (Management Oversight Risk Tree). MORT differed from RCA by assigning causes to common classes of cause shortcomings that could be summarized into a short list. These included work practice, procedures, management, fatigue, time pressure, along with several others. For example: if an aircraft accident occurred as a result of adverse weather conditions augmented by pressure to leave on time; failure to observe weather precautions could indicate a management or training problem; and lack of appropriate weather concern might indict work practices. Because several measures (methods) may effectively address the root causes of a problem, RCA is an iterative process and a tool of continuous improvement. RCA is applied to methodically identify and correct the root causes of events, rather than to simply address the symptomatic result. Focusing correction on root causes has the goal of entirely preventing problem recurrence. Conversely, RCFA (Root Cause Failure Analysis) recognizes that complete prevention of recurrence by one corrective action is not always possible. RCA is typically used as a reactive method of identifying event(s) causes, revealing problems and solving them. Analysis is done after an event has occurred. Insights in RCA make it potentially useful as a preemptive method. In that event, RCA can be used to forecast or predict probable events even before they occur. While one follows the other, RCA is a completely separate process to incident management. Rather than one sharply defined methodology, RCA comprises many different tools, processes, and philosophies. However, several very-broadly defined approaches or "schools" can be identified by their basic approach or field of origin: safety-based, production-based, assembly-based, process-based, failure-based, and systems-based.

Root-Cause Analysis Tools, and How to Use Them

01:10:42
Break/fix is a way of manufacturing life, but how you respond to it is the key difference between merely muddling through and operating a well-oiled process. Whatever process you call it, Corrective Action or CAP or CAPA, the time it takes to find the root cause of any problem is growing. As a result, your production is slowing and your quality is suspect. The quality of your products relies on how quickly you get to the root cause of your production problems. There are many tools available today that can lead us to root cause and then the corrective action, but which one is right for you? In this video Michael Curran-Hays offers a basic understanding of each tool as well as a model for when to them and on what issue. Areas that are discussed: • Common Root Cause Analysis tools in an 8D framework • Human performance module designed to diagnose “Operator Errors” • “Think Beyond the Fix”… Where else could this happen? • Issue escalation model for using the right tool, at the right time, with the right people

Introduction to Root Cause Analysis Video Preview

06:41
Do you have recurring problems that are costing you time and money? Unresolved problems do more than aggravate. They can increase costs, lower quality, and drive customers away. Plus, quality management processes, such as ISO 9001, require organizations to have a corrective and preventive action process in place. Root cause analysis is integral to the success of any corrective action or problem-solving process. Unfortunately, root cause analysis is an often maligned, misunderstood, and misapplied process. Instead of viewing root cause analysis as an opportunity for improvement, many see it only as an admission that things have gone wrong. Root cause analysis should be seen as an opportunity, not a chore. This preview provides a quick overview of Paton Professional's in-depth video Introduction to Root Cause Analysis, hosted by Denise Robitaille. Why-Why Analysis? - Root Cause Analysis Tool

11:53
Why-Why Analysis or which is also known as 5 Whys, is a very popular technique to identify root cause of a problem. This technique is also used in the Analyze phase of the Six Sigma DMAIC (Define, Measure, Analyze, Improve, Control) methodology. By repeatedly asking the question “Why” (five is a good rule of thumb), we can peel away the layers of symptoms which can lead to the root cause of a problem. The technique is explained with examples and practical application to the industry. Using the "5 Why's" to ID the root cause of problems in healthcare.mov

03:10
In this portion of the presentation at the MMGMA (Michigan Medical Group Management Association) conference, Todd Sperl discusses using the "5 Why's" to identify the root cause of problems in healthcare. If we don't ask enough questions, we can come to the wrong conclusions. In one example about x-rays being delayed, the first answer lead us to believe the machine was the issue, however by asking more questions, we find the rad tech is busy answering a phone that should instead be programmed to ring to the clerk's station, which is an easy fix.

Big Data at Audi: Root Cause Analysis in an Automotive Paint Shop Using MLlib

30:03
"The process of painting a car is highly automated, highly complex and depends on various external variables which are, sometimes, difficult to control. Quality standards regarding paint and finish are extremely high at Audi as these are the most visible features of a car to a customer. Today, it takes years of experience to identify the main drivers of paint failures and keep standards accordingly high. For example, different types of paint require different settings regarding process values and application technique. In order to track the level of quality, every single car is inspected by quality assurance and every failure is documented. For documentation, there are more than 200 predefined types of failures available which are used for standardized documentation. While a car is being painted, data is collected from 2,500 sensors. Those parameters include temperatures, humidity, air flow of application robots, energy consumption, state of filters, etc. All of these variables may influence quality positively or negatively. The challenge of supporting process experts with valuable insights into data is solved by storing sensor data in the data lake and processing the data with Apache Spark and Scala on a HDFS cluster. To identify the most important drivers for paint quality for each failure and each layer of paint, 20 random forest models are being trained daily with MLlib. The results are stored in HDFS and visualized with Tableau. This session will give insights into the challenges of big data at an automotive OEM and how the production of Audi benefits from new big data technologies to make their processes more efficient and raise quality standards even higher. To achieve business benefits, Spark is being used along the whole process chain for data ingestion, transformation and training in a productive and completely automated environment. Session hashtag: #SFexp13"

Root Cause Analysis – How it Can Help Your Food Safety Plan

53:17
Food facilities have a variety of mechanisms for identifying food safety failures, including customer complaints, self-inspections, audits, and regulatory visits. The key to success is being able to take action to prevent recurrence of these failures, or at least reduce its likelihood, by identifying and addressing the root cause. During this webinar, you will learn several key methods to help you identify root cause analysis within your facility.
Root Cause Analysis, how science can help us with the investigations

01:14:41
It is commonly known that root-cause analysis can be done with tools like the 5 Whys or Cause-and-Effect Analysis. The advantage of using these tools is that we can organize the information found in the investigation more efficiently. But just identifying potential causes is not a guarantee of effective problem solving, because improper selection of preventions for the causes may result in re-occurrence. The objective of this webinar is to review the application of scientific methods to effectively investigate and identify the causes of food safety problems as well as using science to properly select and manage control methods.
Root Cause Analysis: 8 Points to be effective.

02:05
Another great video made in collaboration with Fabrizio Cireddu. Root Cause Analysis in 8 points. www.leanmanufacturingpdf.com Here the transcript of the text: Tips&Rules Root Cause Analysis 1) Collect correct data: Who, when, where, what, why, how, how many 2) Describe process flow standard and current one (understand deviation) 3) Define potential causes: 6M analysis [Ishikawa] ( Man, Machine, Method, Material, Measurement, Mother Nature) 4) Verify which possible causes it was impacting in Gemba ( go in the real place and check personally) 5) Verify which causes were generating the issue and run for each one 5 Why to define root causes analysis 6) Minimum 3 items to answer with 5 Whys approach: why problem occur? Why was it not detect? Why did system fail? 7) Define Action plan with clear owners and timeline 8) Weekly review of action plan and monitoring results during daily KPI meeting
